Judith Bell, ANP received her B.S. degree in nursing from D’Youville College and went on to achieve her MSN degree in health services administration at Daemen College. Her medical interest areas include adult and geriatric medicine. When Judith isn’t serving patients, she can be found spending time with her family, entertaining and cooking for friends, volunteering, or traveling. She is a member of the American Association of Nurse Practitioners and the Nurse Practitioner Association of New York State.

Jeremy Bellinger, FNP-C graduated from Keuka College with a Bachelors Degree in Nursing and completed his Nurse Practitioner Masters in Nursing Degree from Binghamton University.  He has been working in Skilled Nursing Facilities for many years.

Steven Buslovich, MD, CMD, MS received his B.A. in chemistry from Carnegie Mellon University, and went on to receive his medical degree from the Stony Brook University School of Medicine. He completed his residency in internal medicine at the Yale School of Medicine Greenwich Hospital and continued his education with a geriatrics fellowship at the Icahn School of Medicine at Mt. Sinai Hospital. His medical interest areas include geriatrics, palliative care, and frailty. Dr. Buslovich is affiliated with the Society for Post-Acute and Long-Term Care Medicine and the American Geriatrics Society, and serves as the Cofounder of Patient Pattern, Medical Director for Kaleida Health Skilled Nursing Facilities, Medical Director for post-acute care at Rosa Coplon Living Center at Weinberg Campus, and Medical Director for Lutheran Retirement skilled nursing facility. He is the recipient of the Shark Tank Innovation Award from the American Medical Director Association. His hobbies include skiing, biking, fishing, and cooking.

Katherine Carletta, ANP-C received her B.S. in public health with a minor in relationships and gender studies from the University of Rochester and her M.S. in nursing from the State University of New York at Buffalo. She is certified in adult health and has a special interest in endocrinology, pain management and geriatrics. She is a member of Sigma Theta Tau, the International Honor Society of Nursing, and serves as a nursing representative for a local hospital on its quality improvement team. When she isn’t serving patients, Katherine can be found exercising, golfing, skiing, tap dancing, and spending time with her family.

Dr. Chau received her B.S. in zoology from the University of Michigan and her medical degree from the Wayne State University School of Medicine. She completed her residency in internal medicine at Moses H. Cone Memorial Hospital in Greensboro, North Carolina. She continued her education by completing a geriatric fellowship at the Veterans Administration Medical Center in Buffalo, New York. She is interested in multiple aspects of geriatric care, having worked with patients in sub-acute care units, hospice, and dementia special care units. She serves as the medical director for Beechwood Homes and its affiliates.

Jeffery Facer, MD received his B.S. in physical therapy and completed coursework for his M.S. degree in physical therapy with a specialty track for nonoperative orthopedics and sports medicine at Daemen College. He received his medical degree from the State University of New York at Buffalo’s School of Medicine and Biomedical Sciences, where he also completed his residency in internal medicine. His medical interests include subacute rehabilitation, orthopedics, and wound care, and he is a member of the American Board of Internal Medicine as well as the associate medical director for Family Choice. In his spare time, Jeffery likes to participate in whitewater rafting and hiking.

David Farrugia, MD received his B.S. in biology from LeMoyne College and his medical degree from the University at Buffalo. He completed his residency in internal medicine from West Virginia University. In his spare time, you can catch Dr. Farrugia on the golf course.

Dacia A. Harris, MD completed her undergraduate studies at Howard University with a B.S. degree in Biology and Chemistry, earning membership in the Phi Beta Kappa Honor Society and Beta Kappa Chi National Scientific Honor Society.  She remained at Howard University for her M.D. degree and completed her Internal Medicine Residency through the University of Buffalo.  She has also completed additional training in the Safer/Competent Opiate Prescribing certificate program.  She has a professional interest in Diabetes Management, and enjoys Reading and Traveling.  She maintains the following professional affiliations:  American College of Physicians, AMA, National Medical Association, and American Medical Directors Association.

Deborah Iafallo, ANP, WCC received her associate degree in nursing from Trocaire College, and went on to receive her B.S. in nursing from Daemen College. She continued her education at the University of Buffalo, where she graduated summa cum laude, receiving her M.S. in adult health and her post master’s certification in nursing education. Debby is certified in wound care.

Christine Linder, FNP received her B.S. in nursing from D’Youville College and her M.S. in nursing from the University of Buffalo. She is ANCC certified and has a special interest in geriatrics and palliative care. She is a member of the American Academy of Nurse Practitioners and the Nurse Practitioner Association of New York State.

Leroy W. McCune, MD, is the president of Partners Medical and serves as medical director of several skilled nursing facilities. He received his B.A. in engineering from Dartmouth College and his M.D. from the Geisel School of Medicine at Dartmouth College. He completed his residency in Family Practice Medicine at Lancaster General Hospital in Pennsylvania. He is certified in Hospice and Palliative Care and has a special interest in palliative care and mission medicine. Dr. McCune is a member of the American Academy of Family Practice and the Christian Medical and Dental Association and serves as the president of Storehouse Mission Support. In his spare time, he enjoys aviation, spending time with his family, going to church, traveling, and hiking.

Catherine E Powers, MD completed her undergraduate work at the University of Dayton with a BS Degree in Pre-Medical Studies.  She earned her MD Degree at Wright State University, Boonshoft School of Medicine and completed her Family Practice Residency at the Grand Rapids Family Practice Residency, St. Mary’s Mercy Medical Center.  Cathy enjoys reading, cooking and gardening, and is a member of the following professional organizations:  American Academy of Family Practice and American Medical Directors Association.

Eileen Reilly, MD, received her B.A. in English from Canisius College and her medical degree from the State University of New York at Buffalo. She completed a transitional internship at Evanston Hospital and a family practice residency at the West Suburban Hospital Medical Center, both located in Illinois. She is a diplomate for the American Board of Family Medicine and serves as a member of the American Academy of Family Physicians.

Cynthia Rowles, MSN-FNP received her B.S. in nursing from D’Youville College and graduated with honors as a family nurse practitioner. She has enjoyed working with the older generation for her entire medical career. Cynthia is a member of the medical staff of Sister’s of Charity Hospital, Mercy Hospital and Millard Fillmore Suburban Hospital. Her guilty pleasure is enjoying a bowl of cereal or a Snicker’s bar before bedtime.

Marquita C. Scott, FNP, BC received her BSN and MSN from D’Youville College. She is board certified by the American Nurses Credentialing Center and wound care certified by the National Alliance of Wound Care and Ostomy. She specializes in geriatrics. When she isn’t serving patients, you can find Marquita meditating or going on nature walks.

Patrick Siaw MD, FACP, MRCP (UK) received his medical degree from the University of Ghana. He completed his internal medicine residency at Mount Vernon Hospital, New York Medical College. He also completed a residency in the United Kingdom in internal medicine and geriatrics at the Cardiff Royal Infirmary/University of Wales, leading to board certification by the Royal College of Physicians of London. He specializes in medical education, is a fellow of the American College of Physicians, and serves as a medical director for the Humboldt Housing Nursing and Rehabilitation Center in Buffalo. He is board certified by the American Board of Internal Medicine. He previously served for 15 years as the UB/CHS Internal Medicine Residency Program Faculty Coordinator for Ambulatory Medicine and Clinical Assistant Professor at the University at Buffalo. He received the Best Overall Resident award, was a two-time recipient of the Best Teaching Attending Physician Award at the UB/Catholic Health internal medicine residency program and received a GD Searle Gold Medal Award for providing medical care to the medically indigent. In his spare time, Patrick likes to study history and play soccer.

Cynthia Skalyo, ANP received her BSN from the State University of New York at Plattsburgh, and her MSN at the University of Buffalo. She is certified by the American Association of Nurse Practitioners (AANP) and specializes in geriatrics and palliative care. She is a member of the AANP and the Nurse Practitioner Association of New York State. In her spare time, she likes to sail and spend time with her family.

Alan Smith, MD received his B.A. in biology and chemistry from the University of Rochester and his medical degree from the State University of New York at Buffalo. He completed his residency at Deaconess/BGH Family Medicine and has a certificate of added qualification in geriatrics. He is a member of the Society for Post-Acute and Long-Term Care Medicine and enjoys gardening and kayaking when he isn’t caring for patients.

Brian Snyder, MD, FACP received his B.A. in linguistics and chemistry from Northwestern University, and his medical degree from the State University of New York at Buffalo School of Medicine. He completed his residency in internal medicine at Northwestern University and Evanston Hospital. He is board certified in internal medicine as well as hospice and palliative care medicine and has completed an FDA fellowship in clinical pharmacology. He is a member of the Society for Post-Acute and Long-Term Care, American College of Physicians, and the American Academy of Hospice and Palliative Care Medicine.

Charles Yates, MD received his B.S. degree in management from Case Western Reserve University, his B.S. in pre-medical sciences from Buffalo State College, and his medical degree from the State University of New York at Buffalo. He completed his residency in family medicine at the Niagara Falls Memorial Medical Center and is certified in hospice and palliative medicine. His specialties include frailty, polypharmacy, and geriatric medical transition management. He is a member of the American Academy of Family Physicians, American Academy of Hospice and Palliative Medicine, American Geriatrics Society, AMA, and Medical Society State of New York. In his spare time, he enjoys theatre, playing the tuba, and bicycling.

Cheryl Zachewicz, ANP, -BC, CWS received her BSN and MSN from Daemen College and received training in emergency medicine geriatrics. She is a wound specialist, certified by the American Board of Wound Management. She specializes in post-acute care, emergency room palliative care, wound management, and polypharmacy reduction. Cheryl is a member of the American Nurses Credentialing Center, the American Board of Wound Management, and the American Geriatrics Society. In her spare time, Cheryl likes to travel and spend time with her children.

Margaret “Peggy” Ziobro, FNP received her B.S. degree in nursing from D’Youville College and her FNP from the University of Buffalo. She loves caring for all patient populations, but especially enjoys working with the geriatric population. She is a member of the medical staff at Kaleida Health and Catholic Health.
